This website's general contact form is intended for brief, non-urgent enquiries. It asks for your name, email address, phone number, and an optional short message so the clinic can respond to your request.
Do not send medical records, insurance or payment information, government identification numbers, detailed health histories, or other sensitive information through the general form or ordinary email. Call (415) 841-0115 for instructions when sensitive information is needed.
Information you choose to provide
Information submitted through the general form is intended to help the clinic answer a question or respond to an appointment enquiry. The form is not a clinical intake, patient portal, payment service, or emergency channel.
Please provide only the minimum information needed for a reply. Calling the clinic is the better option if you are unsure whether information is appropriate for the form.
Website and third-party features
A browser ordinarily sends technical information, such as an IP address, browser or device details, and the requested page, to a website and to services it connects to. Some website features may connect to other companies for functions such as anti-spam checks, maps, directions, or reviews. Those companies may process information under their own privacy terms.
The site may store a display preference, such as light or dark mode, in your browser. You can remove locally stored site data through your browser settings. When an interactive map is offered, the street address and a standard directions link provide alternatives.
Your choices and questions
You can choose not to use the general form or other interactive features and contact the clinic by phone instead. Browser controls can limit or remove cookies and local site data, although some features may then work differently.
