Patient Privacy and HIPAA Information

This webpage provides general information and is not the clinic's formal Notice of Privacy Practices. The notice that applies to your care should describe the clinic's actual patient-record practices and your rights under applicable law.

To ask for the current notice or for instructions about a medical-record request, call (415) 841-0115. Do not send medical records or sensitive health information through the general website form or ordinary email.

What a Notice of Privacy Practices explains

When HIPAA applies, a covered health care provider's notice generally explains how health information may be used or shared, the provider's privacy responsibilities, and how a person can exercise applicable rights.

Those rights can include asking to see or obtain a copy of a medical record, requesting a correction or confidential communication, asking for certain limits, requesting an accounting of some disclosures, choosing a representative, obtaining a copy of the notice, and making a complaint. The current clinic notice is the source for the practices, instructions, limits, and contact information that apply to your care.

Website privacy is separate

The Website Privacy Notice describes general website and contact-form privacy. Patient records and information created or maintained as part of care may be governed by different practices and laws. Do not use the general website form to send records, insurance documents, detailed health histories, or payment information.

Copies, questions, and complaints

Call the clinic to ask for the current patient privacy notice, a paper copy, or instructions for a privacy or medical-record question. If HIPAA applies and you believe your rights were violated, the U.S. Department of Health and Human Services Office for Civil Rights explains how to file a health-information privacy complaint.
