Do not use the website or general contact form for an emergency. Call 911 for emergency help. Website information and availability may change, and no particular outcome, timeline, or result is guaranteed.
Acceptable use
Do not attempt to bypass website security, disrupt the site, introduce malicious code, impersonate another person, submit unlawful or harmful material, or use automated tools in a way that places an unreasonable burden on the service.
Do not submit medical records, payment information, government identification numbers, or detailed health histories through the general contact form. The clinic logo and original clinic materials may not be used in a way that falsely suggests affiliation or endorsement. Other material may be subject to separate rights; ask the clinic before republishing website content.

Links to maps, reviews, government resources, and other external websites are provided for convenience. The operator of each external service controls its own content, availability, and privacy practices.
